I understand that this is a general consultation intake form designed so that my stylist at Tiger Lily Beauty Co. can help me choose which New Guest Service option that I'm best suited for (if not already booked), or to help my stylist best prepare for my first visit.

I understand that by submitting this form it does not guarantee that my goal hair can be achieved in one session. This will be determined in person based on the amount of time booked, density/length of my hair, my color history, as well as my time and monetary budget.

I understand that the initial average investment of my first appointment ranges from $150 - $400 depending on my stylist and service choice and that most new client color appointments last 2 - 4 hours.

Maintenance color appointments will depend on my current hair, goal hair, required upkeep, and are generally booked for less time with less monetary investment.
